HOW IT WORKS
You May Qualify
No other job
The family caregiver must be 18 yrs or older and cannot hold another job while caring for their loved one.
Support
You will receive ongoing support from an experience Health Care Coach.
Financial Compensation
You will receive a monthly stipend for doing what you already do, care for your loved one in the home
Medicaid Waiver
The person receiving care must participate or qualify to participate in the CCSP or SOURCE medicaid waiver program.
